This 1-hour course explores aspects of clinical documentation that pose risk, often unknown to the clinician. The use of templates, scribes, and voice-to-text software will be explored. The “Open Notes” provision of the 21st Century CURES Act will also be examined, as documentation in the patient record can lead to mistrust between the clinician and patient. Methods to reduce risk will be provided for immediate application to professional practice.
