Documentation: Best Practices provides strategies to effectively (1) create EHR entries that avoid bias and demonstrate objectivity in an era of open notes, (2) resolve conflict or disagreement with the recommendations of another healthcare professional, and (3) compose record entries that document discord and/or resolution. This course also presents examples of copy-paste function that demonstrate downstream ramifications of engaging in risky documentation behavior.
