Given the complexities and challenges faced by healthcare professionals (HCPs), clinicians must rely on effective and standardized tools for accurate and timely delivery and transfer of patient information. Closing the loop, a frequently used term in the healthcare industry, is an effectively communication model designed to ensure that HCPs exchange vital information in a complete, unambiguous, and timely manner during patient care, handoffs, and transitions to different settings. Closed-loop communication has been shown to team collaboration and minimize medical errors. The goal of this course is to identify strategies for improving communication by closing the loop. Strategies discussed in this activity include the use of Situation-Background-Assessment-Recommendation (SBAR) and the teach-back method.
