This quality improvement intervention engages cardiology care teams to review and improve their current evidence- and guideline-based treatment strategies for patients with SIHD.

Designed to help clinicians assess their clinical practice methods according to current evidence and expert consensus and implement strategies for improving process-related care, including approaches to patient education, symptom assessment and management, and the initiation, monitoring, and modification of disease-modifying therapies.

Neurology

Expanding Success: Improving Alignment With Birth-Cohort–Based HCV Testing and Linkage to Care

This QI initiative, which involves 25 clinics across the US, expands the reach and continues the success of previous QI efforts designed to support clinicians as they change clinical practice patterns to better align HCV testing practices with updated guidelines from the Centers for Disease Control and Prevention and the US Preventive Services Task Force. This initiative also seeks to improve the linkage to specialty care for patients who have a positive HCV antibody test.
