Healthcare Delivery Must Change

The National Quality Strategy has set the expectation that healthcare delivery must change to a system that efficiently deploys available resources and consistently provides patient-centered, evidence-based, and team-based care.

In addition, risk and quality managers look to identify circumstances contributing to medical error. Root-cause analysis and process improvement initiatives can address these challenges and mitigate potentially serious downstream consequences.

We are implementing initiatives that support the ongoing transformation of healthcare from a volume- to value-based system. Our work is changing the delivery of care in primary care clinics, oncology offices, community health centers, specialty disease clinics, emergency departments, and more. We are currently working with more than 25 healthcare organizations across the country to support their quality transformations.

Our efforts are guided by the Institute for Healthcare Improvement Triple Aim, and we seek to develop sustainable quality improvement initiatives that connect clinical performance with process-focused education to improve care, lower costs, and foster a healthier population.

We design and implement quality improvement initiatives that start with a clear aim for improvement, employ multiple Plan-Do-Study-Act cycles, and reinforce education via a carefully planned strategy.

We are committed to starting small, refining as needed, evangelizing success and lessons learned, and scaling our efforts to a broader platform.

EFFICIENCY

Recognizing that demands on clinician time are greater than ever, our QI framework helps clinicians improve practice efficiency and care by reducing fragmentation, unnecessary costs, and inappropriate utilization of healthcare services. We recruit local champions and execute a multifaceted educational strategy to foster efficient process changes.

We launched our first performance- and quality-based educational initiative in 2008 in partnership with The Endocrine Society. Since then, we have implemented more than 20 quality-based initiatives with partners from across the healthcare spectrum. Some of our active and completed projects are highlighted below.
