BALTIMORE, MD—June 4, 2013—Residents of long-term care (LTC) facilities with diabetes experience a higher burden of comorbid disease, more pain, substantial polypharmacy, and an increased incidence of depression; they also require more nursing care. Optimal care of these patients demands a team-based approach by healthcare professionals who are well versed in current guidelines and evidence-based practices.

Med-IQ, an award-winning, ACCME-accredited provider of continuing medical education (CME), partnered with Brookdale Senior Living, Inc. to develop a quality improvement (QI) CME initiative for improving diabetes care in LTC facilities. JAMDA, the official journal of the American Medical Directors Association, published the outcomes of this initiative in its May 2013 issue.

The goal of the Med-IQ/Brookdale Diabetes QI CME initiative was to enhance the quality of care for residents of LTC facilities who have diabetes by:

  • Improving glycemic control
  • Increasing comprehensive diabetes management
  • Reducing fragmented care
  • Empowering patient-care teams to educate patients and their families about the disease
An initial evaluation of care was performed through interviews, confidence surveys, and chart reviews at two Brookdale Senior Living skilled-care communities; then, three live CE/CME workshops were conducted to address the identified areas of need. Workshop attendees reviewed guideline-recommended standards of care with a national diabetes expert and received tools for improving performance in key areas. Chart reviews conducted at two points after the educational intervention revealed changes in clinician performance in key components of diabetes management and critical improvements in patient outcomes. The success of this collaborative initiative between Med-IQ and Brookdale informed the development of a free CME-certified publication featuring key lessons learned from the QI initiative and tools for improvement, which was made available to other LTC clinical staff across the country.

Original research and review articles cover topics such as geriatric medicine, dementia and cognitive impairment, rehabilitation, chronic comorbid conditions, the frail elder, medication management and prescribing issues, multi-resistant organisms and infectious diseases, falls prevention, assisted living risks and challenges, health policy, outcomes evaluation, and guidelines for administrators, physicians, and staff who work in LTC and rehabilitation sites.
