Overview: This accredited CME Express publication reviews the advantages and disadvantages of home hemodialysis (HHD) that should be considered when devising clinical management strategies for patients with end-stage renal disease. It also discusses key criteria for identifying patients who are appropriate candidates for HHD as well as patient- and provider-associated barriers to HHD use.

Target Audience
This activity is intended for nephrologists and physicians who care for patients who receive home hemodialysis.
 
Statement of Need
Hemodialysis is the most common renal replacement therapy for patients with end-stage renal disease (ESRD) and is initiated when patients exhibit signs and symptoms of renal failure. Home hemodialysis (HHD), which is used by only 3.5% of patients with ESRD, takes place in the comfort of the patient’s home and provides more flexible scheduling for dialysis, including conventional, short daily, and nocturnal protocols. When using HHD, patients benefit from greater independence and improved outcomes, including better blood pressure control, improved quality of life, and longer survival. However, due to American clinicians’ long-standing inexperience with HHD, the majority of patients with ESRD are unaware of HHD as a treatment option. Clinician and patient education on HHD eligibility requirements, as well as the advantages and disadvantages of home treatment, is needed so that appropriate American patients with ESRD can achieve the long-term benefits associated with HHD.
